Description Where did you undertake your learning in practice? Who supervised your learning? Who else was present e.g. other healthcare professionals? Which types of patients did you see? Were they in your area of practice? If not, then justify the generic learning outcomes from the session(s). 3. Contribution What activities did you observe? What activities did you undertake? What feedback from colleagues and/or patients did you receive? 4. Learning What did you learn during this time in practice? What discussions did you have with the supervisor about clinical decision making? What do you feel about transitioning to your new role? 5. Action plan the first diary show be a novice watch learn and observe second should be competent taking a lead show knowledge skills learning from peers and last should show proficiency ( works at 2 pages per reflection)
