The Antecedents and Consequences of Top Management Fraud

Are rules and regulations established to protect against management fraud effective? If so, why is fraud still so prevalent? What else can be done to prevent/deter fraud? How can you prove intent? If someone doesnt intend to commit fraud but nevertheless their actions or inactions lead to fraud, should they be held responsible? Do you think society, as a whole, is to blame for top management fraud?
