Do a comprehensive investigation report on an injury that was cause by Citric Acid w/sodium hydroxide (showing 10 ph at the time) that was splashed into the eye of an employee while draining some fin fans in unit 14 at Marathon Refinery. Was the employee using the right PPE at the time of the incident? The report shows he was using safety glasses but the JSA calls for a face shield. Did the other workers involved properly relay messages to the affected employee to let him know the valves were about to be opened and did the supervisor properly handle the incident and the reporting procedures. The incident allegedly happed at 2:30 p.m. on 10/29/2019 and the affected employee left the refinery an hour later but was not reported to Marathon safety until the following day (is this a violation)? They safety showers were not used, no pre plan on jsa, no barricade around job with tags. Ashton (supervisor on shift) allowed the affected employee to leave the facility and drive himself home and not report incident to Marathon.
