The goal of this paper is to explore the research evidence that supports the use of a particular health promotion or disease prevention strategy and the effects of implementing this strategy to a population. The paper should also consider the cost effectiveness of the strategy, and consider the feasibility of integrating the program into the normal provision of care. You should consider what policy (e.g. federal, state, local) changes would be needed to enable the most effective implementation. Paper should reflect a community level health promotion or prevention strategy and not a disease intervention Instructions 1.) Write an introduction that clearly defines the evidence based practice issue and its significance to healthy outcomes. Incorporate Healthy People 2020 and preventative task force guidelines as appropriate 2.) Summarize the findings from your review of the literature on the topic. Use current (< 5 years) research, evidence based practice and articles. 3.) Identify strengths, weaknesses, barriers and opportunities of implementing the intervention in the primary care setting.
