Using the best aspects of the community/population identify elements that are key to the success of the plan that will be established.2. Leading community/population problems based on data gathered.The teaching plan should be one that is relevant to the community and has the best opportunity for success and sustainability over time with reasonable resources.3. Indicate these factors in your plan with the rationale. The plan should focus on one problem to be resolved. The role of nursing should be clearly identified in the plan as well as any interdisciplinary activities to meet the population needs.Application of the Social Determinants of Health to the priority problem4. Identify how the social determinants of health affect the planning for the project. What are the positive and negative aspects of the social determinants that may alter the strategies used for this project?Review of literature focused on the priority problem and interventions that have evidence of effectiveness for similar communities or populations.5. All plans must include a review of the scholarly literature related to the population and the strategies that have been developed to alleviate the priority problem. The approaches to the resolution to the population/community problem may be new and innovative but should have a rationale for that approach.Plan and the timing — all plans must have a schedule for the development, implementation, and evaluation of the project.6.
